Health Insurance and Sliding Fees
While Lower Lights Health is not a free clinic, all patients are cared for regardless of their ability to pay.
Insurance:
We accept most major health insurance plans including Medicaid (CareSource, Molina, Buckeye, Paramount and UnitedHealthcare Community Plan), Medicare and commercial insurance.
Please note that all co-pays are due at check-in. We accept cash, check and credit card (Mastercard, Visa or Discover).
Sliding Fee Scale:
For patients without health insurance coverage, fees for services are based on a sliding fee scale, determined by household size and income. If your household income is at 200% or less of the federal poverty level, you will be eligible for a sliding fee scale.
You will need to bring in proof of income when you come for your visit. Your most recent income tax return is the best proof of income, but you may also bring in pay stubs, W-2s, Social Security documents and 1099s. Please provide income for the whole household as well as how many people live in your household.

Items to Bring to Your First Visit
Our doctors, nurses and staff wish to serve you with the highest level of care. To help us do this, we ask that you bring the following with you to your first visit:
- Photo ID
- Proof of income (only required if applying for a sliding fee)
- Insurance card (if applicable)
- A list of the medications you are currently taking
- Immunization records for children
- Your co-pay (Co-pays are required for each provider service. For example, an additional co-pay will be required when a patient sees a doctor for a medical visit, a counselor for behavioral health care and the dietitian for nutrition care.)
Your appointment time will start when you arrive at registration. It may be 30 minutes before you see your doctor or nurse.
We ask that you be an active participant in your care by asking questions and following your treatment plans. If you follow these expectations, our doctors, nurses and staff will be able to enhance your quality of care. It is our pleasure to serve you.

FAQs
Anyone can become a patient. Whether you have insurance, no insurance or are somewhere in between, everyone is welcome at Lower Lights Health. Our doctors, nurses and staff are committed to treating patients with respect, dignity and compassion regardless of race, color, sex, national origin, disability, religion, age, sexual orientation and ability to pay. Sliding fees are available for those who qualify.
We encourage patients to make appointments in advance. By scheduling one to two weeks in advance, you will have a wide selection of appointment times available. If you have an urgent medical need, some same day appointments are available for existing patients. Appointments may be scheduled for new patients within the same week depending on availability.

If you need to cancel your appointment, please cancel by calling 614-274-1455 (option 1) at least 24 hours in advance.
If you are more than 15 minutes late for your appointment, there is a chance your doctor or nurse may not be able to see you. Further, not showing up for scheduled appointments may result in not being able to be scheduled for future appointments.
Yes, we practice comprehensive health care and therefore require all patients to see a medical doctor prior to seeing the dentist, optometrist, dietitian or behavioral health counselor.
All of our doctors, nurses and staff are paid full-time and part-time employees. All clinicians are board-certified in their specialty. We currently offer primary care level services.
Yes, when scheduling your appointment, you may specify which doctor or nurse you would like to see.
All co-pays/sliding scale fees and insurance cards (if applicable) must be provided at the time of service or you could be subject to rescheduling.
The length of an appointment varies with medical services. In general, it’s always best to have plenty of extra time in case you are referred to do lab tests, for example. Most patients should plan for one hour from check-in to check-out.
Yes, we have doctors, nurses and staff who speak other languages, and we offer interpretation services via phone and iPad as well. In addition, there are two on-site Spanish interpreters at the Health Center location in Franklinton. Please make your language request when scheduling your appointment and we will do our best to fulfill your need. This website may be read in other languages by selecting a language of your choice in Google Translate.

Lower Lights Health provides cutting-edge, high quality primary care to all. Integrated whole person care is available for all ages and includes dental, vision, behavioral health services, pharmacy and on-site laboratory services for your convenience. Our dedicated team of board-certified physicians and nurse practitioners focus on quality care and patient satisfaction. In addition, those with private insurance help us offset the cost of treating those who are uninsured or underinsured.
To obtain a copy of your medical records, you must complete and sign an Authorization to Release Medical Record form. Please allow up to seven days to process your request.
