Provider Relocation Notification

Dear Valued Patient,

Thank you for choosing to receive your care at Lower Lights Health. We know that patients have many options to choose from and we value your partnership. Continuity of care, access, and relationships are core to our mission. We do need to let you know that your current healthcare provider, Tim Stephens, will begin providing services at our Franklinton Central Lower Lights location in April.

Our Franklinton Central site is located at 777 West State Street, Suite 201-A, Columbus Ohio 43222. If you’ve not visited this location before, you’ll find patient, visitor and handicap parking in Parking Garage #1, next to Medical Office Building 2, across from the Mount Carmel Emergency Room. The entrance for this garage is at the intersection of West Town Street and South Davis Avenue. All transportation service vehicles may pick up and drop off patients and visitors under the awning of Medical Office Building 2.

Please call us at (614) 274-1455 to schedule an appointment with Tim at his new location, if you do not already have one scheduled, and do not hesitate to use our Walk-In Provider at the Health Center (1160 W. Broad St.) as needed.
