Provider Change Notification

Dear Valued Patient,

This letter is to let you know that your current medical provider, Christine Smith, is no longer practicing at Lower Lights Health as of this October. While we’re grateful for her service to you and our other patients, we have identified a new medical provider who will see you on future visits to Lower Lights Health’s Cooper Road location. Our goal is to continue your care with the least amount of interruption. Your new medical provider is Nurse Practitioner John Boudouris.

Here is a special note from John:

Education: The Ohio State University, Master of Science in Nursing, Master of Public Health, Bachelor of Science in Zoology

Professional Background: Ohio Living Nurse Practitioner, Ohio Health Registered Nurse, Ohio Department of Health Epidemiology Investigator

I aim to help patients determine their health goals and to support their efforts in achieving them. I look forward to meeting you!

During the evaluation with your new provider, you will spend time reviewing your history and current medications. Your new provider wants to understand your medical background to determine the best therapy for each medical concern you might have.

Please call us at (614) 274-1455 to schedule an appointment with John Boudouris if you do not already have one scheduled, and do not hesitate to use our Walk-In provider at the Health Center as needed as well.
