Provider Change Notification

Dear Valued Patient,

Thank you for choosing to receive your care at Lower Lights Health. We know that patients have many options to choose from and we value your partnership. Continuity of care, access, and relationships are core to our mission. We do need to let you know that Tim Stephens MSN, APRN, FNP-C, will be moving to another Lower Lights location in April. While we are grateful for Tim’s service to you and our other patients, we’re very pleased to introduce you to a new medical provider who will see you on future visits to our Marysville location. Our goal is to continue your care with the least amount of interruption. Your new medical provider is Physician Assistant Sam Barnard.

We’d like to share some information about Sam Barnard’s experience and vision for patient care:

Education: Kettering College- Master of Physician Assistant Studies, Cedarville University- Bachelor of Arts in Athletic Training

Memberships: American Academy of Physician Associates (AAPA)

Professional Background: I am a primary care physician assistant with prior experience in sports medicine

Personal statement: I am thrilled to join the team and provide care to both English and Spanish speaking patients! I believe in a holistic approach to healthcare, emphasizing preventive medicine, spiritual care, patient education, and collaborative, patient-centered decision-making. In my down time, I love spending time with my family and friends, serving at our local church, and playing and watching soccer.

During the evaluation with Sam, you will spend time reviewing your history and current medications. He wants to understand your medical background to determine the best therapy for each medical concern you might have.

Please call us at (614) 274-1455 to schedule an appointment with Sam Barnard if you do not already have one scheduled, and do not hesitate to use our Walk-In Provider at the Health Center (1160 W. Broad St.) as needed.
