Provider Change Notification

Dear Valued Patient,

Thank you for choosing to receive your care at Lower Lights Health. We know that patients have many options to choose from and we value your partnership. Continuity of care, access, and relationships are core to our mission. We’re very pleased to introduce you to a new bilingual medical provider who will see you on future visits at our Health Center location. Our goal is to continue your care with the least amount of interruption. Your new medical provider is Nurse Practitioner Daniela Salas MSN, APRN, FNP-C.

We’d like to share some information about Daniela Salas’s experience and vision for patient care:

Education: Ohio University – Bachelor of Science in Nursing; University of Cincinnati – Master of Science in Nursing

Memberships: American Association of Nurse Practitioners (AANP)

Professional Background: Before I started working at Lower Lights, I worked as a registered nurse in postpartum and pediatrics at Northside Hospital in Atlanta, Georgia and Nationwide Children’s Hospital. I also worked as a family nurse practitioner at Mount Carmel.

Personal Statement: As a family nurse practitioner, I enjoy taking care of patients of all ages from infants to the elderly. I love serving the Hispanic community and look forward to caring for Hispanic patients in Spanish. I also enjoy spending time with my patients and listening to their concerns, so that together we can work on achieving their health goals.

During the evaluation with Daniela, you will spend time reviewing your history and current medications. She wants to understand your medical background to determine the best therapy for each medical concern you might have.
