Lower Lights Health is proud to announce that our German Village location has officially earned Patient-Centered Medical Home (PCMH) Recognition from the National Committee for Quality Assurance (NCQA), which stands as the most widely adopted and respected evaluation program for patient-centered primary care in the United States.

This recognition is more than a certificate on the wall. It represents a deep, organization-wide commitment to delivering care that puts patients first, strengthens relationships between patients and providers, and supports healthier communities across Columbus.

What PCMH Recognition Means

The Patient-Centered Medical Home model redefines what health care can look like. Instead of fragmented visits and rushed interactions, a PCMH focuses on strong relationships between patients and their care teams, as well as continuous quality improvement.

National research has shown that practices recognized as PCMHs see improved patient outcomes, better patient experiences, increased staff satisfaction, and reduced overall health care costs.

A National Standard of Excellence

NCQA’s PCMH program is the gold standard in this field. More than 10,000 practices and 50,000 clinicians across the country have achieved PCMH Recognition. Additionally, over 95 different organizations nationwide support NCQA-recognized practices in some capacity through financial incentives, transformation support, care management, learning collaboratives, or MOC credit.

By earning this recognition, Lower Lights German Village joins an elite group of practices across the country that have demonstrated excellence in patient-centered primary care.
