As you get older, it’s important to start taking more responsibility for your health—and we’re here to help. Think of this as your next step toward independence!
💬 Filling Out Forms
Sometimes, you’ll be asked to fill out a quick questionnaire before your visit. This gives you a chance to share how you’re doing—physically, mentally, and emotionally. It also helps us understand what you might need, without putting everything on your parents.
🔒 Private Conversations
We think it’s great when teens and parents talk openly. But during your visit, you’ll also get time to talk one-on-one with your doctor. What you say in private stays private (aka confidential), unless there’s a serious safety concern. Your provider is there to listen and support you.
📱 MyChart = Your Chart
If you haven’t already, get your own free MyChart account and download the app on your phone. You can:
- Check your test results
- Message your doctor
- See upcoming appointments
- Stay in the loop with your health
🧠 Be Informed, Be Confident
Taking care of your health means asking questions and knowing where to find good info. Our clinic’s website is a great place to start. And remember—we’re always here to guide you.

AWKTALK.org: Medically accurate, teen friendly sexuality health web site. There is a ton of information on this site for teens and parents including content about puberty, consent, contraception and pregnancy. There is a section now devoted to parents and trusted adults where there is an e-learning curriculum.

Got Transition: A website providing resources and guidance on pediatric-to-adult health care transition.
Kids Thrive 585: A program focused on advancing access to transition support for youth with special needs.
National Institutes of Health: Offers resources and information on adolescent health and the transition to adulthood.
OkaySo: This is an app that teens can download from the APP Store. The APP allows teens to ask anonymous questions about reproductive health and a NCH health educator will answer the questions within a day.
