Do You Need Help Enrolling for Medicaid?

We will be hosting Medicaid Enrollment & Assistance Hours! These events will be held in the patient lobby at the Health Center.

Wednesdays beginning April 19 through June 28
from 10:00 am–5:00pm

Services available include: presumptive eligibility and Medicaid re-enrollment

To better assist you, please bring:

• Valid State of Ohio photo I.D.
• DOB of all family members applying for Medicaid
• Current Medicaid card
• Income verification (pay stubs)
• Social Security Card
