Provider Change Notification

This letter is to let you know that your current medical provider, Sarah Johnson, will no longer practice at Lower Lights Health starting on August 16, 2024. While we’re grateful for her service to you and our other patients, we have chosen a new medical provider who will see you on future visits to Lower Lights Health’s Health Center location. Our goal is to continue your care with the least amount of interruption. Your new medical provider is Family Nurse Practitioner Trinean Wamah. Here is a special note from Trinean:

Education: Indiana Wesleyan University, MS, FNP, University of Cincinnati, BSN

Membership: American Association of Nurse Practitioners (AANP)

Professional Background: Family Nurse Practitioner, Walk in Urgent Care; Field Nurse Practitioner, United Healthcare-Optum; RN, Mount Carmel MIMCU

Personal Statement: I see my work as a nurse practitioner as my service to people and to God. I strive daily to provide care that is holistic, balanced, and compassionate with the belief that quality care is due to EVERY individual. I look forward to joining the team at Lower Lights Health in serving the community.

During the evaluation with your new provider, you will spend time reviewing your history and current medications. Your new provider wants to understand your medical background to determine the best therapy for each medical concern you might have.

Please call us at (614) 274-1455 to schedule an appointment with Trinean Wamah, MS, CNP if you do not already have one scheduled, or to obtain your medical records. If you have not enrolled in MyChart, please consider doing so to virtually schedule appointments, communicate with your provider, and submit prescription refill requests.
